Disability and FMLA Forms
We ask that you submit your request at your earliest convenience and your paperwork will be returned to you via regular mail within 5-7 business days. We apologize for the inconvenience, however, it will be your responsibility to submit your forms/letters to the appropriate third party. We cannot fax or forward these forms on your behalf. Please be informed that your forms will be completed with a return to work date of 6 weeks post-delivery for a vaginal delivery and 8 weeks post-delivery for a Caesarean section. Any additional time off, if necessary, will be dictated by your physician.
