  • MyChart will allow you to self-schedule important pre-op health appointments.
  • MyChart will send reminders for your pre-op appointments, you can complete the pre-op Joint Replacement Discharge Planning Questionnaire and/or I-PSS (males only), and other tasks to complete before your surgery date.
  • MyChart gives you access to your test results, allows you to communicate with your providers, pay your bill, and helps you to coordinate your healthcare needs.

Surveys:

An important e-mail from the American Joint Replacement Registry (AJRR) will be sent to you. You MUST Complete these two online surveys before your surgery date:

Veterans Rand 12 (VR-12) and HOOS, Jr (hip)/KOOS, JR (knee) surveys will tell your physician how you were feeling before your orthopaedic surgery. Your answers will help you and your physician better understand how you are doing. This information will also help improve care fo patients undergoing similar orthopaedic procedures. Your answers to the survey questions are protected and secure. The AJRR system will only share your information with your physician's office. Complete your physician's surveys via the internet using the American Joint Replacement Registry (AJRR) secure patient portal (website).

EACH SURVEY SHOULD ONLY TAKE 5-10 MINUTES TO COMPLETE

  • You will receive these two surveys to complete in three different intervals: Pre-op, 3-months Post-op, 1-year Post-op. The surveys will tell your physicians how you were feeling before and how you are feeling after your orthopaedic surgery.
  • For those patients who do not have internet capabilities, please notify the Joint & Spine center of GBMC. These surveys can be printed, completed, and mailed to the Joint & Spine Center of GBMC.
